Management of Nonviable Early Pregnancy

This virtual coffee break was created by the Clinical Training Center for Sexual and Reproductive Health to help providers diagnose, counsel, and treat clients with nonviable intrauterine pregnancy.  It is intended for Title X program clinical providers (MD/NP/PA/CNM), nurses, and other staff who may assess and triage clients with early pregnancy symptoms.

After completing the activity, participants will be able to:

  • Describe the process of determining viability in early pregnancy.
  • Define common terms associated with early pregnancy loss (EPL).
  • Discuss considerations for counseling clients with non-viable pregnancies.
  • Identify three EPL management options and describe contraindications, risks, benefits, and patient counseling considerations for each.

This Virtual Coffee Break e-learning is intended for Title X clinical providers, nurses, and other staff who provide pregnancy testing and counseling to clients.

After completing this activity, participants will be able to:

  1. List the types of pregnancy tests and their uses based on their human chorionic gonadotropin (hCG) measurements and thresholds
  2. Describe the process of determining gestational age
  3. Discuss additional considerations for determining gestational age outside of the typical 28-day menstrual cycle
  4. Identify the actions necessary for evaluating and counseling a client with a positive pregnancy test

Nondirective Counseling for Pregnancy Options in Title X Clinics

The National Clinical Training Center for Family Planning (NCTCFP) partnered with the Adolescent Health Initiative to produce this resource for local sites to encourage care teams to consider how staff and providers in varied roles can create a climate that fosters adolescent-friendly nondirective counseling for pregnancy options following the Title X 2021 final rule.

Sparks are free, ready to use, and include a PowerPoint presentation, a facilitator script, and follow-up materials. They are designed for providers or staff to deliver in 15-30 minutes at staff meetings. Sparks can be facilitated by providers or staff in any role, and are specifically designed to “spark” discussion and reflection among a multidisciplinary audience. Discussing the 2021 Title X Final Rule

The NCTCFP speaks with Jessica Swafford Marcella, MPA, the Deputy Assistant Secretary, Office of Population Affairs and Director of the Office of Adolescent Health, to discuss the new Title X Final Rule, which went into effect on November 8th, 2021, including changes to options counseling and telehealth services.

In October 2021, HHS published the Title X Final Rule “Ensuring Access to Equitable, Affordable, Client-Centered, Quality Family Planning Services.”

The 2021 final rule revokes the 2019 final rule, “Compliance With Statutory Program Integrity Requirements”, in its entirety and readopts the 2000 final rule, “Standards of Compliance for Abortion-Related Services in Family Planning Services Projects” with revisions to ensure access to equitable, affordable, client-centered, quality family planning services for all clients, especially for low-income clients. This document, published by the Office of Population Affairs, provides a summary of essential provisions within the 2021 rule.
